Please print and complete the Initial Visit Information, HIPAA Notice of Privacy Practices and Consent for Psychotherapy Services forms and bring them to your first appointment. If we have chosen to conduct therapy via telehealth, please sign and mail me the telehealth consent form. These forms can be downloaded from the links below. If you are unable to print the forms please call me, as these forms must be completed prior to our first session.
